Case management at ERC/Insight is a collaborative, process-driven practice model rooted in seamless coordination of care and communication.

A case manager is assigned to each patient during the clinical assessment phase of admission. The case manager’s sole responsibility is to source, organize and monitor treatment records to enable clinical teams—ours and yours—to provide the very best care to patients and families.

Case managers engage with referring providers to discuss treatment goals and determine communication preferences—how often they want to receive treatment updates, but also what specific information they want to know related to their patients in our care.

At the time of discharge, case managers ensure outpatient providers have comprehensive discharge plans, informing them about the treatment received at ERC/Insight and empowering them to usher their patient into the next phase of recovery.
 
Laura Feilner is a case manager in Eating Recovery Center’s Child and Adolescent Eating Disorder Program in Denver, Colorado. She has served in this role for 3 ½ years, and previously she worked as a Behavioral Health Counselor within the same program. While Laura acknowledges that excellent case management fosters consistency of experience for outpatient providers, she also explains another meaningful benefit:
 
“The support and collaboration of our skilled case managers means that families don’t have to worry about the minutiae and administrative tasks of treatment, like sourcing medical records and compiling paperwork at admission and discharge. Our work enables families to focus on recovery and engage fully in the ERC treatment model, including family therapy, psychoeducation, skills groups and peer support. And when they discharge, their providers already know what they’ve done here and how to help the family move forward into recovery.”
